I, hereby agree to my child to be apart of the Outlet LLC. I agree to the learning as well as what’s being implemented into my child. I agree to my monthly payment of $180. If my child is absent due to sickness, vacation, or personal reasons I understand that I cannot receive a refund. My monthly payment is due each month on the designated date agreed upon. I understand that the Outlet LLC is not responsible for pace as well as how quickly a child learns but understand that the fundamentals of what’s being implemented will take time as well as parent involvement. If learning mats or curriculum is ruined AFTER it was placed in possession of parent, the Outlet LLC will not be held responsible UNLESS issue was made note of as being an error on the Outlet LLC end. All scheduling must be made in advance, unless it is an emergency. The Outlet LLC cannot be held responsible for any information that I did not give.

To pay my child’s payment, I agree to submit all payments via website. If I miss a payment I would be charged an additional $10 a day through separate invoicing. I hereby agree to each thing that was listed.
